Abstract
PRAMEHA PRATISHEDHA: AYURVEDIC STRATEGIES FOR PREVENTING AND MANAGING TYPE 2 DIABETES MELLITUS

ABSTRACT

Prameha, a group of urinary disorders described in Ayurvedic classics, closely resembles Type 2 Diabetes Mellitus (T2DM) in modern medical science. Characterized by excessive, turbid urination (prabhuta mutrata, avila mutrata) and metabolic disturbances, Prameha is primarily a Tridoshaja Vyadhi with Kapha predominance. Among its various forms, Apathyanimittaja Prameha aligns most closely with T2DM and is largely attributed to lifestyle and dietary indiscretions. The increasing global prevalence of T2DM, particularly in India, regarded as the "Diabetic Capital of the World", necessitates exploration of integrative and holistic approaches like Ayurveda for prevention and management. This review article explores Ayurvedic strategies for the prevention (pratishedha) and management (chikitsa) of Prameha, with a focus on individualized, stage-specific, and constitution-based therapy. The therapeutic approach varies depending on the type of Prameha (Sahaja vs Apathyanimittaja), the strength and constitution of the patient (Krusha vs Sthula), and the stage of disease progression.
The treatment protocol includes four essential components:
1. Nidana Parivarjana & Pathya Ahara and Vihara - elimination of causative factors and diet and lifestyle modifications
2. Samshodhana Chikitsa - purification therapies
3. Samshamana Chikitsa – palliative therapies
4. Rasayana Chikitsa - rejuvenation therapies
This paper also underscores the importance of patient education and long-term adherence to dietary and lifestyle changes for sustainable disease control. Ayurvedic classification of Prameha allows for a nuanced, tailored therapeutic approach that goes beyond symptomatic treatment to address the root causes and systemic imbalances involved in T2DM. The integration of Ayurvedic principles with modern diabetic care can enhance patient outcomes, reduce complications, and offer a cost-effective and holistic healthcare solution. In conclusion, Ayurvedic management of Prameha offers a multifaceted approach that targets the pathophysiology, lifestyle causes, and long-term consequences of Type 2 Diabetes. By combining detoxification, dietary regulation, herbal therapy, and rejuvenation, Ayurveda provides a preventive and curative pathway well-suited for the growing global diabetes burden.
