UNUSUAL PRESENTATIONS OF VULVAR CANCER

ABSTRACT
Background: Vulvar cancer is an uncommon gynaecological malignancy. The disease often presents with atypical symptoms, leading to diagnostic delays and advanced-stage presentation. Case Presentations: We present two rare and atypical cases of vulvar squamous cell carcinoma (SCC) in postmenopausal women. The first case involved a 65-year-old female with a history of vulvar trauma more than one year ago managed by self local medication which was confirmed on multiple wedge biopsy as locally advanced keratinizing SCC and managed with primary radiotherapy. The second case involved a 74-year-old female with a flat elevated base vulvar growth confirmed on biopsy as SCC and lymph node involvement. Following chemotherapy, imaging revealed pyometra and further evaluation uncovered focal non-keratinizing SCC in the cervix, suggesting either synchronous cervical carcinoma or contiguous spread. Conclusion: These cases highlight the diagnostic complexity and clinical variability of vulvar cancer presenting with atypical symptoms or histories. The possibility of unusual metastatic pathways should also be considered in advanced cases.
