COMPREHENSIVE REPORT ON HYPERTENSION MANAGEMENT UNDER NPCDCS AT AAM SUB-CENTRE, RAIPUR

ABSTRACT
Hypertension is one of the most significant non-communicable diseases (NCDs) contributing to global morbidity and mortality and is a major risk factor for cardiovascular diseases, stroke, and renal disorders. In India, its prevalence is steadily increasing due to epidemiological transition, rapid urbanization, changing dietary patterns, sedentary lifestyle, and psychosocial stress. The rising burden of hypertension has posed a significant challenge to the healthcare system, particularly in rural and underserved populations.To address this growing public health concern, the Government of India has implemented the National Programme for Prevention and Control of Cancer, Diabetes, Cardiovascular Diseases and Stroke (NPCDCS), which focuses on early detection, standardized treatment, and community-based management of NCDs. The present study evaluates hypertension management under NPCDCS at an Ayushman Arogya Mandir (AAM) Sub-Centre in Raipur.The study integrates field observations, Community-Based Assessment Checklist (CBAC)-based screening data, and National NCD portal reports to assess screening coverage, prevalence, treatment practices, and service delivery among individuals aged 30 years and above. The findings indicate that community-based screening conducted by ASHAs and ANMs has significantly improved early detection of hypertension. The use of standardized treatment protocols and digital monitoring systems has enhanced treatment initiation and patient tracking.However, despite these improvements, gaps remain in community awareness, treatment adherence, regular follow-up, and consistent availability of essential medications. Lifestyle-related risk factors such as physical inactivity, unhealthy diet, and stress continue to contribute to the burden of uncontrolled hypertension.In conclusion, NPCDCS has strengthened hypertension management at the primary healthcare level, but further improvements in infrastructure, awareness programs, and patient-centered care are essential to achieve effective long-term control and reduce the burden of hypertension.
