PHARMACOKINETIC DETERMINATION OF GLIMEPIRIDE IN TYPE-2 DIABETES MELLITUS PATIENTS: HERBAL DRUG INTERACTION

ABSTRACT
Objective: The present study was undertaken to determine the possible beneficial effect of poly-herbal formulation (Mehagni) on metabolic and pharmacokinetic parameters of glimepiride in type-2 diabetes patients. Materials and Methods: Sixty-four, type-2 diabetic patients on glimepiride therapy were randomized into two different groups; 32 patients taking glimepiride tablet (2 mg once daily) and 32 patients taking a combined therapy [Glimepiride (1mg tablet/day) + mehagni-500mg (2 tablets twice a day)] for 12 weeks. Results: At 12 weeks after the combined therapy, significant reduction in FBS, PPBS and HbA1c levels was observed in Group A1 and Group A2 patients from baseline. In addition, the level of low density lipoprotein was significantly decreased [118.02±9.5 to 112.55±2.4 (p<0.05)]. Decreased level of triglycerides [163.26±3.83 to 152±5.8 (p<0.05)] and total-cholesterol [162.43±3.41 to 156.80±4.70 (p<0.05)] were also found in this study. The level of high density lipoprotein was significantly increased [43.15±6.58. to 47.40±3.45 (p<0.05)]. Furthermore, the blood samples were collected at 0.5, 1, 2, 3, 4, 6, 8 and 12 h time points for pharmacokinetic study. Cmax of glimepiride at second hour of combination therapy was increased significantly (197.37±12.27), compared to glimepiride treatment (159.71±12.15). The area under curve was altered significantly and no patient has experienced hypoglycemia. Conclusions: Based on these results, we conclude that administration of poly-herbal formulation in combination with glimepiride has significant effect in reducing the dose of glimepiride to achieve better therapeutic response with minimal side effects. The co-administration of poly-herbal formulation with glimepiride showed better glycemic control and lipid lowering effect.
