PREVALENCE OF OBESITY AND ITS ASSOCIATION WITH CVD RISK FACTORS IN NEPALESE PATIENTS WITH TYPE 2 DIABETES

ABSTRACT
Obesity plays an important role in causing insulin resistance and increasing CVD risk in patients with Type 2 Diabetes. This is due to generation of systemic inflammation by expressing cytokines like TNF-α and IL-6. Furthermore, presence of other CVD risk factors like high blood pressure, dyslipidemia and atherogenic life style adds on to the risk of CVD in these patients. This study was taken to observe frequency of obesity and its correlation with various modifiable and non modifiable CVD risk factors in Type 2 Diabetes patients. Retrospective data collected from files of 103 adults with Type 2 Diabetes attending medical opd at BPKIHS, Nepal were analysed. 74.8% (n=77/103) of patients had obesity as defined by BMI>25. Higher percentage of females were obese than males. Obesity was found to be highest among middle aged patients (40-60 yrs of age), n=54/103, 80.6% as compared to elderly (>60 yrs of age) and young patients (<40 yrs of age). Higher percentage of patients who smoked and drank alcohol were obese as compared to those who didn’t smoke and consumed alcohol viz n=16/21, 76.2% Vs n=46/65, 70.8% and n=18/21, 85.7% Vs n=47/69, 68.1% respectively. Uncontrolled hyperglycemia was more frequently seen with obese as compared to non obese, 68.8% Vs 26.9% p <0.05. Also, BMI showed positive correlation with TC, LDL, TG and HbA1c and negative correlation with HDL. Prevalence of obesity was high among type 2 diabetics. Obesity correlated well with other CVD risk factors in these patients. Hence it can be concluded that encouraging weight management along with regular treatment for Type 2 Diabetes is very important to decrease occurrence of CVDs in future.
